What are the the numbers in the last line of the smsd_stat/status file (16, 0, 0)?

The "16" is number of sent SMSes as far as I can understand.
Do not know about the others yet.

Yes, the "16" is number of sent messages and this number includes all SMSes after you installed smsd with statistics enabled. This number is stored in the file /var/spool/sms/stats/GSM1.counter.
Next values are number of failed and number of received messages (including status reports), after the smsd was last started.
As your modem does not tell what is the bit error rate, you could use a modem setting signal_quality_ber_ignore = yes.

However, I do not see that last 0 increase when receiving an SMS.... Any ideas?

No ideas currently. I have to test it myself on Raspbian with 3.1.15 from repository.

With Raspbian Stretch 2017-09-07 and smstools 3.1.15 from repository the counter increased properly while smsd was running. Notice that other counters than "sent" starts from zero when stats_interval is reached.
